Overview: As a Hospice RN Liaison, you will have the opportunity to educate healthcare professionals about hospice services and provide end-of-life solutions committed to clinical excellence, compassion, and dignity. • Ensure effective and timely intake and referral processes to meet patient and family needs
• Complete the admissions process for referred patients, including emergency cases
• Provide accurate and timely information regarding hospice services to inquiring parties
• Act as a patient advocate in evaluating and developing care plans
• Call on various account types and offer in-services and education on the benefits of hospice Requirements:
  • Compassionate and empathetic RN with a hospice heart
  • Active RN license in the relevant state(s)
  • Minimum 2 years of healthcare experience
