• Certification Details
• BLS - American Heart Association only
• License/Certificate Verification - Required for clinical positions
• MMR - Required, with potential follow-up boosters
• Fit Test - Completed within 1 year of start updated annually
• Hepatitis B - Proof of full series or at least two vaccines completed, or signed declination
• Tdap - Proof required within past 10 years
• Influenza Vaccine - Proof of vaccine from Oct-March or approved exemptions
