Job Description:

Seeking a per diem / prn nurse RN Home Health Hospice for a per diem / prn nursing job in Akron, Ohio.
We are looking for a compassionate and skilled Registered Nurse to provide end-of-life care to patients in their preferred environment.

  Responsibilities
  
  • 
    
  • Plan of Care: Evaluate patient treatment and medical condition, individualize the plan of care, and make adjustments as needs change.
    
  • Minimize Patient Risk: Update doctor orders in patient chart and administer medication as prescribed by the physician.
    
  • Continuity of Care: Manage multi-disciplinary care, promote continuity of care with appropriate admissions, transfers, and discharges.
    
  • Family Educator/Advocate: Counsel patient and family on disease/progression, incorporate patient and family in development of plan of care.
    
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
    
  • Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs.
    
    Requirements
    
    • 
      
    • Registered Nurse with current license in the state of employment.
      
    • Minimum one-year experience as an RN in an acute care setting.
      
    • Hospice experience is desired.
      
    • Valid driver's license and auto insurance in your name as a driver.
      
    • Capable of all physical demands.
